Connor’s Hope Goods and Services Application

This application is a request for financial assistance from Connor’s Hope, Inc. The applicant must be 18 years of age or older and a legal dependent of his or her parent/legal guardian and a resident of Ohio. The application request must come from/be completed by the applicant’s parent/legal guardian and/or transition coordinator or support administrator. Please complete all sections of the application truthfully. If information is found to be misleading, false, or inaccurate it will result in disqualification of the application. Applicant needs to provide the latest ETR, IEP or ISP. Provide detailed explanation of the desired reason/use for the goods and services. Please know that we welcome any applicant to think outside the box for support for any goods or services that will bring the young adult joy.
